This person, supervised by the Program Supervisor or Lead Case Manager, works in conjunction with other personnel in order to provide community based Mental Health Support Services including family and individual case management, crisis intervention, and life skills training to the client. Responsible for clinical documentation of services, including completion of assessments documents and service plans designed to meet the goals and objectives of clients. This position works in the community and a flexible schedule is required, including on-call crisis intervention.
II. Essential Job Functions:
provides case management and training for individuals in specific life and social skills areas
provides case management services
provides transportation to appointments as needed
maintains documentation pertaining to client medical records, including
completion of assessments and service plans
responsible for the coordination and integration of services
performs other duties as assigned
arrange and attend clinical supervision
III. Qualification Standards:
Minimum qualifications of a QMHP-E means a person in the human services field who is trained and experienced in providing psychiatric or mental health services
At least a bachelor’s degree in a human service field or special education from an accredited college without one year of clinical experience or
At least a bachelor’s degree in a nonrelated field and is enrolled in a master’s or doctoral clinical program, taking the equivalent of at least three credited hours per semester and is employed by a provider that has a triennial license issued by the department and has a department and DMAS-approved supervision training program.
Clinical experience means providing direct services to individuals with mental illness, mental retardation or receiving gerontology or special education services. It includes supervised internships, practicums, and field experience.

IV. Knowledge, Skills, & Abilities:

Certification in First Aid and CPR.
Valid Va Driver’s License and proof of Insurance. Must maintain physical and emotional health to satisfactorily meet the demands of this position; In addition, evidence of a valid Virginia driver’s license is required.
